To purchase 3D-printed orthotics, customers must complete a contact form available on the FIT4 website.
Upon submission, FIT4 will connect the customer with the closest participating clinic based on the details provided.
The individual clinic is solely responsible for prescribing, manufacturing, and providing orthotics tailored to the customer’s needs.
3. Responsibility of Clinics
Each clinic operates independently and is fully responsible for:
Assessing the customer’s suitability for 3D-printed orthotics.
Manufacturing and delivering the orthotics.
Managing any refunds, cancellations, or aftercare inquiries related to the orthotics provided.
FIT4 does not oversee the clinics’ operations and disclaims all responsibility for any aspect of the orthotics’ quality, fit, or effectiveness.
4. Refunds, Cancellations, and Aftercare
All requests for refunds, cancellations, adjustments, or aftercare must be directed to the clinic that provided the orthotics.
FIT4 is not liable for any disputes, damages, or claims arising from transactions or interactions between customers and clinics.

Customers are responsible for following any instructions or guidelines provided by the clinic regarding the use and maintenance of orthotics.
